Avatar Lifting Therapy Terms and Conditions – Client Consent Form
I understand that this procedure is purely elective. What to Expect:
• Depending on the area of your face or body being treated, the procedure is well-tolerated and, in some cases, virtually painless, feeling only a mild prickling sensation.
• The professional will apply a topical anesthetic to your skin prior to treatment to reduce any pain and discomfort.
• Your skin will be pink and brown in appearance, much like an avatar, the following 4 days.
• Swelling will occur on the next two days when done on eyes.
• Your skin may feel warm, tight, and itchy for a short while. This should subside in 12-48 hours.
Possible Side-Effects:
• Side effects or risks are minimal with this type of treatment and typically include flaking or dryness of the skin with scab formation that will fell off totally within 7 days.
• Hyper-pigmentation (darkening of certain areas of the skin) can occur due to sun or heat exposure, after the scab have fell off, is very rarely and usually resolves after 2 months with the application of a hyper – pigmentation product.
• If you have a history of cold sores, this procedure may cause flare ups. NEED MEDS
- Temporary redness and mild-sunburn effects may last up to 4 days.
Freckles may temporarily lighten or permanently disappear in treated areas.
Other potential risks include crusting, itching, discomfort, bruising, infection, swelling, and failure to achieve the desired result. Permanent scarring (less than 1%) is extremely rare.
The benefits and risks of the procedure have been explained to me, and I accept these benefits and risks. The nature of my medical or cosmetic condition has been explained to my satisfaction as have been any substantial or significant risks of harm. I am also aware of and accept the risk of rare and unforeseen complications which may not have been discussed and which may result from this treatment.
I have had the opportunity to ask questions and seek clarification of this procedure and its alternatives including no treatment and my questions have been answered satisfactorily. I understand the following contraindications listed below and will notify my provider if any of the following apply to me:
• Active infections – viral, fungal, bacterial
• Rashes, warts, skin cancer
• Active acne
• Immune-suppressed patients
• Skin-related autoimmune disorders
• Pregnant or breast-feeding
• Recent ablative dermal procedures
• Rosacea
• Uncontrolled Diabetes
• Keloid
No alcohol 48 before or 4 days after the procedure.
Avatar Lifting Therapy Seminar Terms and Conditions Policy
The cost of the Isa Skin Care Avatar Lifting Therapy Seminar in the US is $2,500 per student.
In this case withJen Cartagena there was not charge any fees.
If you can’t attend your scheduled seminar, you may attend one seminar anywhere in the US for up to 1 year from the date of the original scheduled seminar at no additional seminar cost.
All Isa Skin Care practice machines are issued free of charger to take with them at the end of the seminar to all students who complete the Avatar Lifting Therapy Seminar and are issued during the seminar.
This seminar includes a Certificate by Isabel Brown, 63-page book, 2-day practice with live models, practice machine to take home with you, full support after the seminar and lunch. These practice machines are not FDA approved. There is no warranty expressed or implied on the machines. Upon signing below, you agree to hold Isa Skin Care, LLC and or Isabel Brown and or their assigns harmless from all legal actions unconditionally and forever.
You agree not to use social media or any other vehicle to slander or undermine Isa Skin Care, LLC and or Isabel Brown to include the Avatar Therapy.
In addition, you agree to hold all information received from Isabel
Brown, Isa Skin Care, LLC and or information received from the Avatar
Seminar confidential for your individual use only. You agree not to reproduce any materials from the Avatar Seminar. You further agree not to teach the Avatar Seminar or Avatar Therapy to anyone without written permission from Isabel Brown and or her assigns. You further agree, that during the seminar, you will not use cell phones and or video equipment. We will give a 15-minute breaks for cellphone and restroom use. You agree not to consume any form of alcoholic beverage at least 48 hours before the seminar and at least one week after the seminar. Alcohol interferes with the healing process.
The Seminar location is from 10:00 to 4:00.
